The clinic's operation is based on the same team-based strategy for care which has been successfully implemented at most of the other 50 medical student run free clinics that exist nationally.

First year and second year students are paired with third and fourth year students in order to obtain vitals, focused histories, and physicals. The students present the patient to an attending physician and propose a care plan including pertinent labs. With physician approval, they help to implement this plan.

During a typical day at the clinic, clinic volunteers pursue health maintenance for chronic conditions such as hypertension or diabetes, serve as a primary contact for non-emergent medical illnesses, and provide health education and various social services for patients with significant barriers in health care access. There are also specialty clinics, including a women's clinic, children's vaccination drive, and health fair.

The clinic's student directors lead a committed student organization that consists of chairs for all the separate free clinic committees, and other committee members. The student organization already includes more than 60 medical students on fundraising, interdisciplinary, supplies, public relations, community liaison, web services, and education committees. The clinic is governed by a Board of Directors that consists of faculty members of Vanderbilt University School of Medicine, community members, and the clinic student directors.
